This is a Home Health Physical Therapist opportunity in Seaside OR and surrounding areas. The facility is a Home Health setting operating under Providence Portland Home Health North Coast. The system does a good job grouping clients to minimize drive and windshield time with an occasional maximum radius of roughly 60 miles (6090 minute total drive time) per day.

• May visit up to 6 patients per day; 30 productivity points standard (1:1 ratio).
• Complete Oasis assessments as required.
• Utilize EPIC Remote Client for Home Health (on laptop) and Rover (phone app) for charting.
• Provide skilled home health physical therapy services to patients across the assigned service area.

• 1 year of Skilled Home Health Experience required.
• Proficiency with Oasis documentation (required).
• Proficiency with EPIC Remote Client for Home Health and Rover (phone app).
• Must have own transportation.

• PT License required.
• BCLS (Basic Cardiac Life Support) certification required.

• Minimum 1 year of Skilled Home Health Experience required.
