Kace Premier Medical Talent is recruiting a Physical Therapist for a skilled nursing rehabilitation opportunity in Hayward California . Help patients improve mobility rebuild functional abilities and achieve individualized rehabilitation goals.

Responsibilities:
Evaluate strength balance gait mobility and functional limitations.
Develop individualized treatment plans with measurable goals.
Provide therapeutic exercise gait training balance training and functional mobility interventions.
Monitor progress and adjust treatment plans as appropriate.
Educate patients and caregivers on safe movement and continued recovery.
Collaborate with rehabilitation and nursing colleagues and support discharge planning.
Complete accurate and timely clinical documentation.

Qualifications:
Graduate of an accredited physical therapy program.
Current California PT license or eligibility to obtain licensure before starting.
Strong assessment clinical reasoning and documentation skills.
Ability to work effectively with an interdisciplinary care team.
