What Youll Do

As a Home Care Physical Therapist youll provide individualized evidence-based care in patients homes including:

• Evaluating patients mobility strength balance and functional abilities
• Developing and implementing personalized treatment plans to improve movement function and independence
• Providing therapeutic exercises gait training balance training and fall prevention strategies
• Assessing home environments and recommending safety modifications or assistive devices
• Educating patients and caregivers on treatment plans home exercise programs and injury prevention
• Collaborating with physicians and interdisciplinary care teams
• Completing accurate timely electronic documentation
