Per Diem
Summary
The Per Diem Physical Therapist provides skilled hospice care services to adult and geriatric patients across a variety of settings including private homes assisted living communities and inpatient facilities.
As a key member of the interdisciplinary care team the Physical Therapist delivers patient- and family-centered care that promotes comfort safety mobility and dignity at end of life. This role supports patients in remaining in their home setting whenever possible while receiving high-quality compassionate care.
Qualifications
- Minimum of one year of clinical experience as a licensed Physical Therapist required.
- Graduate of an accredited physical therapy program.
- Home health or hospice experience strongly preferred; or an equivalent combination of education and experience that provides the necessary skills to perform the role.
- Ability to work independently and manage time effectively in a field-based environment.
- Strong clinical assessment problem-solving and organizational skills.
- Excellent interpersonal and communication skills.
- Proficiency with electronic medical records (EMR) systems preferred.
- Valid drivers license and reliable transportation required.

The Pay Range listed is the anticipated hiring range of this position. Precise pay rates are determined through consideration of one or more of the following: education training relevant experience and work location. There are some cases in which the pay rate could exceed the range listed on this job post. Care Dimensions also offers a comprehensive benefits package including employer-sponsored health insurance paid time-off tuition reimbursement student loan paydown supplemental insurance options and more which adds to the companys generous total rewards package.
Pay Range
$58 - $58 USD
