Compensation: $60 - $69 per hour
We are seeking a Physical Therapist to join a collaborative skilled nursing facility (SNF) team in Concord CA. This opportunity is ideal for a clinician who enjoys helping patients improve mobility strength balance safety function and independence through individualized evidence-based care.
Evaluate patients and develop individualized plans of care based on functional goals.
Provide evidence-based therapeutic exercise mobility balance gait and functional training.
Monitor patient progress and modify treatment plans as clinically appropriate.
Educate patients families and caregivers on safety and home programs.
Complete accurate and timely clinical documentation.
Collaborate with physicians nursing rehabilitation professionals and the interdisciplinary team.
Graduate of an accredited Physical Therapy program.
Active CA Physical Therapist license in good standing or eligibility for licensures.
Current BLS/CPR certification as required by the employer.
Strong clinical reasoning communication documentation and teamwork skills.
Experience in the applicable setting preferred; qualified clinicians at varying experience levels are encouraged to apply.
