What Youll Do
Under the supervision of a Licensed Physical Therapist you will:
- Deliver physical therapy treatments according to established plans of care
- Help patients improve mobility strength balance function and independence
- Communicate patient progress and concerns with the supervising Physical Therapist and interdisciplinary care team
- Collaborate with physicians nursing staff caregivers and families to support patient outcomes
- Participate in care planning and interdisciplinary team meetings as appropriate
- Provide education and training to patients families and facility staff
- Complete timely electronic documentation treatment notes and daily billing activities
- Assist with discharge planning and continuity of care
- Support restorative and functional maintenance programs within the facility
- Maintain productivity and documentation standards while delivering high-quality patient-centered care
Qualifications
- Graduate of an accredited Physical Therapist Assistant program
- Associate degree as a Physical Therapist Assistant
- Current PTA license in the state of practice
- New graduates are encouraged to apply
- Strong communication teamwork and clinical problem-solving skills
- Passion for delivering exceptional patient-centered care in a Skilled Nursing Facility setting
