Pleasant Hill Post Acute is seeking acompassionate motivated and dependable Physical Therapist Assistant (PTA)to join our rehabilitation team on aPRN basis.

The PTA works underthe direction of the supervising Physical Therapist to provide high-quality rehabilitation services that promotemobility independence safety and overall quality of lifefor our residents.

Position Responsibilities

• Provide skilled physical therapy treatments and interventions as directed by the supervising Physical Therapist.

• Assist residents in achieving individualizedmobility and functional goals.

• Complete timely accurate and compliant clinical documentation.

• Maintain a safe and clean treatment environment while following infection control and safety protocols.

• Communicate regularly with the supervising Physical Therapist regarding resident progress goals changes in condition discharge planning and care coordination.

• Collaborate with nursing and other members of the interdisciplinary team.

• Participate in Resident Care Conferences and rehabilitation meetings as needed.

• Assist with training and support ofRestorative Aides.

• Educate residents families and caregivers regarding treatment plans home exercise programs safety techniques fall prevention and maintenance programs.

• Assist with recommendationsfor durable medical equipment and adaptive equipment to promote resident safety and independence.

• Provide staff educationand in-service training as needed.

• Report equipment concerns and help ensure rehabilitation equipment remains safe and functional.

• Participatein quality assurance and performance improvement activities.

• Maintain strict confidentiality of resident and company information.

• Develop positive and professional relationships with residents families coworkers and healthcare professionals.

Qualifications

• Associate Degree from an accredited Physical Therapist Assistant (PTA) program required.

• Current California Physical Therapist Assistant license required.

• Skilled nursing or post-acute rehabilitation experience preferred.

• Strong communication organization and teamwork skills.

• Dependable andflexible with the ability to work on an as-needed basis.

• Commitment to providing compassionate resident-centered care.
