Working under the direction of a Physical Therapist you will:

• Provide physical therapy treatments in patients homes

• Carry out established plans of care

• Provide therapeutic exercise gait balance transfer and mobility training

• Help patients improve strength endurance and functional mobility

• Reinforce home exercise and fall-prevention programs

• Educate patients and caregivers

• Monitor progress and report changes to the supervising PT

• Coordinate with the interdisciplinary care team

• Complete accurate and timely documentation using WellSky

• Active Virginia Physical Therapist Assistant License Required

• Graduate of an accredited PTA program

• Valid Drivers License and reliable transportation

• Strong patient care and communication skills

• Ability to work independently during home visits while following the PTs established plan of care

• Home Health Experience Preferred but Not Required

• WellSky Experience is a Plus
