Compensation: $50.00 per hour or more depending on experience

We are seeking a Physical Therapist Assistant to join a supportive skilled nursing facility (SNF) rehabilitation team in Petaluma CA. Working under the supervision of a licensed Physical Therapist you will carry out individualized treatment plans and help patients progress toward meaningful functional goals.

Implement treatment plans established by the supervising Physical Therapist.

Provide therapeutic exercise gait balance mobility and functional training.

Monitor patient response and communicate progress to the Physical Therapist.

Educate patients and caregivers within the established plan of care.

Complete accurate and timely documentation.

Collaborate with the rehabilitation and interdisciplinary care team.

Graduate of an accredited Physical Therapist Assistant program.

Active CA PTA license in good standing or eligibility for licensures.

Current BLS/CPR certification as required by the employer.

Strong communication documentation teamwork and patient-care skills.

Experience in the applicable setting preferred.
