Physical Therapist Assistant FT, Day

• Implements and manages a specific and comprehensive treatment plan and participates in patient discharge planning:

• Administers patient treatments within the plan of care (POC) outlined by a licensedphysical therapist and progresses patients as indicated

• Demonstrates basic knowledge of diagnoses/conditions seen in physical therapy

• Interacts with others involved in the patients care to incorporate an interdisciplinarytreatment approach for coordinated quality care

• Demonstrates awareness and use of proper safety measures regarding the patientsothers in the home and self

• Assesses patient response to treatment and modifies goals and treatments as indicated

• Provides information regarding follow-up services which may include a referral to othercommunity resources

• Participates in patient and family education process.

• Supports and provides educational material to patient/families for support groups as wellas community resources

• Provides education and training to patients families and caregivers regarding diagnosesprogress and long-term functional outcomes

• Teaches counsels and communicates with the patient family or caregiver educationalinformation related to the patients diagnosis/condition

• Complies with departmental policies for documentation.

• Documentation is in compliance with chart review audit components

• Documents all aspects of assessment planning treatment and discharge in the computerin a timely manner

• Follows POC as defined by the primary therapist and documents the patients progress toward goals

• Daily treatment records are noted in the computer on a daily basis and are clear andprofessional in terminology.

• Appropriate abbreviations and medical terminology are utilized

• Obtains re-evaluations as required by S. C. Physical Therapy Practice Act

• Performs other duties as assigned.

Supervisory/Management Responsibilities

• This is a non-management job that will report to a supervisor manager director or executive.

Minimum Requirements

• Education - Associate degree inAPTA Accredited Physical Therapy Assistant Program

• Experience - Two (2) years PTA experience

• SC PTA

• A valid drivers license; an acceptable motor vehicle record as defined by the Acceptable Motor Vehicle Record (MVR) Chart; and proof of auto insurance.

• CPR Certification
