Job Description

What you will be doing:

Implement individualized physical therapy treatment plans developed by the Physical Therapist.
Assist with participant assessments including vital signs range of motion strength testing and functional mobility.
Deliver therapeutic interventions including therapeutic exercise gait training balance training muscle re-education and functional mobility training.
Assist participants in improving performance with activities of daily living (ADLs).
Coordinate participant treatment schedules therapy visits and related documentation.
Collaborate with physicians therapists nurses and interdisciplinary team members to provide comprehensive care.
Maintain accurate clinical documentation in accordance with regulatory and organizational standards.
Provide therapy services in both outpatient center and home-based settings as needed.

Experience you will need:

Associates Degree from an accredited Physical Therapist Assistant (PTA) program.
Valid Physical Therapist Assistant (PTA) License or proof of license eligibility.
Knowledge and experience providing therapy services to frail or medically complex older adults.
1 to 2 years of PTA experience preferred.
Experience in one or more of the following settings: Outpatient Rehabilitation Acute Care Inpatient Rehabilitation Skilled Nursing Facility (SNF) Home Health.
Experience treating adult and/or geriatric populations.
Strong communication teamwork and patient education skills.
Candidates with only pediatric experience will not be considered.
