Pay-range : 1yr: $39.95- 12: $54.10
Shifts : M-F 8-4:30 (but can be a little flexible with that). Requires 2 weekend days per month.

The Physical Therapist (PT) provides high-quality patient-centered care by managing the complete process of therapeutic care of an assigned group of patients in coordination with the physician and the patient care team. The Physical Therapist completes patient assessments and participates in developing and implementing individualized care plans focusing on goal achievement patient engagement and positive patient experience. The Physical Therapist provides or directs a majority of the patients physical therapy to support patients throughout the continuum of care.

What you will do in this role:
- You will provide evaluations and treatment interventions within the scope of practice established by law and professional code of ethics.
- You will assess and evaluate therapeutic/rehabilitative/functional status and participate in the development of the total plan of care.
- You will provide purposeful and skilled therapeutic care at patients home using procedures and techniques to produce a change in condition/function consistent with the physicians plan of care.
- You will instruct patient/family/caregiver in the use of therapies exercises therapeutic appliances and/or prosthetic devices to be used outside of therapy sessions.
- You will prioritize caseload determine daily staffing needs and coordinate assignments/handoffs.
What qualifications you will need:
- Open to New Grads (Experienced is not required)
- Bachelors Degree in Physical Therapy REQUIRED
- State Licensed Physical Therapist
- Basic Cardiac Life Support must be obtained within 30 days of employment start date
