Employment Type: Per Diem Field Visits local to Salem Windham and surrounding Southern New Hampshire communities
We are seeking experienced and compassionate Licensed Practical Nurses (LPNs) to join our Southern New Hampshire home care team on a per diem basis. This position offers independence flexibility and meaningful one-on-one care for patients who prefer to remain in the comfort of their homes throughout Salem Windham Derry Londonderry Pelham and surrounding communities.
This opportunity already has an established part-time route available and is ideal for nurses seeking flexible supplemental income while making a meaningful impact in their local community.
The LPN will provide skilled nursing visits focused on promoting recovery maintaining stability and enhancing quality of life. This is an ideal role for nurses who thrive in autonomous settings and want flexible scheduling with competitive per-visit compensation.
Conduct in-home patient assessments medication management and wound care.
Monitor and document changes in patient condition and report to supervising RNs or physicians.
Educate patients and caregivers on treatment plans medications and home safety practices.
Maintain accurate timely documentation using electronic charting.
Collaborate with multidisciplinary care teams to ensure coordinated and effective patient care.
Competitive per-visit pay averaging $50/hour based on actual time spent per visit.
Established route with approximately 23 days per week already available.

Current and active LPN license in the State of New Hampshire (required; New Hampshire is not a compact nursing state).
Valid drivers license and reliable transportation.
Prior experience in home care long-term care rehabilitation or community nursing preferred.
