Alvita Care is seeking compassionate reliable and professional Licensed Practical Nurses (LPNs) for per diem floor staffing opportunities in Patchogue NY. SCHEDULE
- Most available shifts are 3 PM11 PM on weekdays and weekends.
- Per diem positions with flexible hours. Full-time or part-time hours may be available based on facility staffing needs.
POSITION DETAILS
- Position: Licensed Practical Nurse (LPN)
- Employment Type: Per Diem
- Location: Patchogue Suffolk County NY
- Setting: Facility Floor Staffing
- Pay Rate: $35$40/hour based on experience and assignment
RESPONSIBILITIES
- Provide quality nursing care and support to residents.
- Administer medications and treatments within the scope of the LPN license.
- Monitor residents conditions and report changes to the appropriate healthcare professionals.
- Maintain accurate and timely documentation using the facilitys charting system.
- Follow facility policies procedures and infection control protocols.
- Collaborate with other healthcare professionals to promote resident safety and well-being.
- Assist with other nursing duties as assigned within the scope of practice.
QUALIFICATIONS
- Valid New York State LPN license required.
- Previous nursing experience preferred.
- Long-term care or facility experience preferred.
- Strong communication and documentation skills.
- Authorization to work in the United States.
- Updated medical and vaccination records or willingness to obtain them.
- Completion of required drug screening and health documentation.
- Successful completion of Alvita Cares onboarding process before starting an assignment.
- Ability to meet facility scheduling and staffing requirements.
