Founded in 1982 as a non-profit organization by members of the recovery community in Mesa Arizona Community Bridges Inc. has held as its principal mission maintaining the dignity of human life for more than 40 SummaryThe Continuing Care Peer is an individual with lived experience in recovery and who provides community-based outreach support navigation and care coordination for individuals experiencing behavioral health challenges substance use homelessness or crisis situations. This position provides peer support outreach care planning and resource linkage both onsite and in the community. This Continuing Care Peer supports individuals during deflection encounters sobering services and transitions to appropriate levels of care while ensuring followup advocacy and continuity of servicesSkills/Requirements:High school diploma or GED six months of recovery from substance use and/or mental health in outreach peer support homeless services or behavioral health license (valid and in good standing) 39-month motor vehicle record Support Specialist certification from the Oregon Health Authority required or able to be obtained within 90 days of is growing and expanding our services!We are experiencing tremendous growth currently.
