We are seeking a compassionate motivated Physical Therapist to provide individualized treatment in a skilled nursing environment. This opportunity is ideal for a PT who enjoys working collaboratively with residents and an interdisciplinary care team.

• Evaluate residents functional abilities mobility balance strength and rehabilitation needs.
• Develop individualized treatment plans based on clinical assessments and resident goals.
• Provide therapeutic interventions to improve walking transfers coordination and daily function.
• Monitor resident progress and adjust treatment approaches as appropriate.
• Document evaluations treatment sessions progress notes and discharge recommendations accurately.
• Collaborate with nursing occupational therapy speech therapy and other team members.
• Educate residents families and caregivers on exercises mobility techniques and safety strategies.
• Follow established clinical standards and applicable documentation requirements.

• Current Michigan Physical Therapist license in good standing.
• Degree in Physical Therapy from an accredited program.
• Strong clinical judgment and effective communication skills.
• Ability to work independently while contributing to a multidisciplinary team.
• Previous experience in skilled nursing post-acute rehabilitation or geriatric care is preferred.
