• Conduct in-home client assessments and reassessments.
• Develop review and update individualized Plans of Care.
• Conduct supervisory and quality-assurance visits.
• Follow up with clients and families after new starts of care.
• Monitor client satisfaction and identify concerns before they escalate.
• Recognize and report changes in clients condition safety mobility behavior or care needs.
• Assist with caregiver-client matching based on skills personality and client needs.
• Work closely with the Scheduler/Care Coordinator regarding client and caregiver concerns.
• Coach caregivers on Plans of Care client expectations professionalism and performance.
• Address client and family concerns promptly and professionally.
• Support new starts of care and help ensure a smooth transition.
• Maintain accurate and timely documentation of assessments visits incidents and follow-ups.
• Escalate significant client or caregiver concerns appropriately.
• Maintain confidentiality and professional boundaries.

• Current California CNA certification or LVN license preferred.
• Minimum 2 years of senior-care experience preferably in home care senior living hospice home health skilled nursing or a related setting.
• Experience with client assessments care coordination supervisory visits caregiver supervision or case management preferred.
• Experience with dementia Alzheimers disease chronic conditions and mobility limitations.
• Strong communication documentation organization and problem-solving skills.
• Confident communicating with clients families and caregivers.
• Comfortable coaching caregivers and addressing performance concerns.
• Valid California drivers license reliable transportation and ability to travel throughout the service area.

• Experienced CNA/senior-care professional: approximately $27$30/hour
• Experienced LVN with strong care-management experience: approximately $31$36/hour
