Job Description:

Palliative Care APRN wanted to join a diverse IDT that includes Physicians chaplaincy social work RN and pharmacy diverse practice includes an inpatient palliative medicine consulting service outpatient palliative clinic.

Responsibilities:

Reviews clinical information and completes new consultations and follow ups in a timely manner working with the interdisciplinary team (physicians social workers and chaplains)
Act as an integral part of the treatment team determining goals of care for patients with cancer-related or life-limiting chronic illnesses; collaborate with palliative care physicians regarding symptom management needs.
Provides extensive education regarding chronic diseases and how they impact quality of life.
Communicates with treatment team including other providers on changes to plan of care.
On call coverage requiredfor palliative care

Qualifications:

Demonstrated skills and knowledge as a Nurse Practitioner in area of expertise: One (1) year nurse practitioner experience preferred. One (1) year hospice or palliative nursing experience preferred. Excellent observation problem solving skills mathematical skills and nursing skills. Knowledge of community professional and education resources.

Excellent communication skills written and verbal amongst professionals
