Job Summary

Position Summary

Oversee the daily operations of a post-acute care practice in Houston TX. Practice providers work in skilled nursing facilities rehabilitation centers assisted living communities and long-term care settings.

Key Responsibilities

Operational Management

  • Collaborate with administrative and clinical leadership to ensure seamless communication between providers and facilities.
  • Monitor operational KPIs staffing coverage scheduling accuracy documentation compliance and service delivery metrics.
  • Develop and implement operational policies workflows and performance standards to improve efficiency and patient care coordination.
  • Support and mentor administrative and operational staff.

ProviderOnboarding & Support

  • Coordinate onboarding for new providers including physicians nurse practitioners physician assistants and ancillary staff.
  • Through partnership with Recruitment and Credentialing facilitate provider orientation EMR access training schedules workflow education and facility introductions.
  • Serve as a primary operational point of contact for new providers during onboarding and transition periods.

FacilityOnboarding & Relationship Management

  • Conduct facility readiness assessments and ensure operational alignment prior to go-live dates.
  • Assist leadership with staffing plans workflow delegation and operational training initiatives.
  • Promote a collaborative patient-centered and accountability-driven culture.
  • Coordinate implementation activities including workflow setup communication protocols billing integration documentation processes and provider scheduling.
  • Support ongoing facility engagement issue resolution and service optimization initiatives.

Qualifications:

  • 3 years of management experience in a healthcare organization required
  • Excellent communication customer service and critical thinking skills
  • Proficiency with Microsoft Office
  • Strong attention to detail and organizational skills
  • Ability to handle confidential information in accordance with HIPAA regulations
  • Ability to prioritize and manage multiple tasks with minimal supervision
