What Youll Do

• Perform comprehensive occupational therapy evaluations and functional assessments
• Develop individualized treatment plans based on patient goals and clinical needs
• Deliver skilled interventions focused on activities of daily living (ADLs) and instrumental activities of daily living (IADLs)
• Address cognitive perceptual sensory upper extremity and functional mobility deficits
• Recommend adaptive equipment environmental modifications and safety strategies
• Educate patients families and caregivers to support successful outcomes
• Collaborate with nursing physicians rehabilitation professionals and facility leadership
• Supervise and support Certified Occupational Therapy Assistants (COTAs) as appropriate
• Participate in interdisciplinary care plan meetings and discharge planning discussions
• Maintain accurate timely and compliant documentation in accordance with regulatory requirements

Qualifications

• Bachelors Masters or Doctorate degree in Occupational Therapy
• Current Ohio Occupational Therapist license in good standing
• Skilled nursing geriatric or post-acute rehabilitation experience preferred
• Strong clinical reasoning and problem-solving abilities
• Excellent communication and interpersonal skills
• Commitment to patient-centered care and positive clinical outcomes
