Job Summary
Duties:
Performs initial and on-going assessments of patients in both short- and long-term residential programs.
Develops and implements individualized therapy intervention designed to promote healing restore function maintain function compensate/adapt to impairments for optimal function or prevent complications arising from an injury/impairment.
Identify needs for and make referrals to other disciplines or for durable medical equipment to address the comprehensive needs of the patient.
Instruct educate and train patients/caregivers in the skills and functions essential for promoting patient independence and productivity in order to diminish or correct pathology.
Assesses a patients home environment for safe community reintegration.
Performs administrative tasks such as maintaining patient records documentation via electronic medical records in a timely manner writing reports and making telephone calls/consultations as needed.
Supervise Certified Occupational Therapist Assistants per state guidelines.
Communicates continually with COTAs and other rehabilitation team members to create the most effective individualized treatment outcomes for patients.
Provide hands on one on one treatment to patients including but not limited to:
lifting (50-100 lbs) transferring assisting with Activities of Daily Living in a safe manner with stamina and endurance.
Therapists also participate in community reintegration outings and patient home evaluations to promote optimal recovery.
Qualifications:
Demonstrates eligibility to work for an employer in the U.S.
Must be licensed in the State of llinois and/or eligible for licensure as required by the state practice guidelines.
Must be a graduate of an occupational therapy curriculum accredited by AOTA & ACOTE standards.
Current Basic Life Support certificate or ability to obtain within 45 days of the date of hire.
Previous experience within a Rehabilitation setting preferred but not required
Ability to multitask and move between responsibilities in a fluid manner.
Must demonstrate excellent communication skills both verbal and written English Communications for safe and effective patient care and to meet documentation standards.
Requires great organizational and time management skills

Ability to relate and work effectively with others.
