Kace Premier Medical Talent is recruiting an Occupational Therapist for a skilled nursing rehabilitation opportunity at a hospital site in Hayward California Help patients regain practical skills and improve independence through individualized occupational therapy.

Responsibilities:
Assess patients ability to perform daily living activities.
Develop treatment plans addressing functional limitations and rehabilitation goals.
Provide self-care training therapeutic activities and appropriate functional interventions.
Assess adaptive equipment needs and teach safe effective strategies.
Educate patients and caregivers.
Collaborate with the interdisciplinary team on care and discharge planning.
Reassess progress and complete accurate clinical documentation.

Qualifications:
Graduate of an accredited occupational therapy program.
Current California OT license or eligibility to obtain licensure before starting.
Strong assessment clinical reasoning and documentation skills.
Ability to collaborate effectively across disciplines.
