Job Description Summary:

The Occupational therapist will complete OT screens comprehensive OT evaluations guided by standard standardized assessments and the OT process provide 1:1 OT treatment for underserved groups and populations. This position will help individuals maximize occupational participation performance in meaningful daily activities including but not limited to emotional regulation skills self-care activities instrumental activities of daily living work and educational occupations and activities involving social participation.

What Youll Do:

  • Collaboratively address the occupational needs of individuals groups and populations via provision of quality and outcome oriented occupational therapy services by assessing and interpreting individuals and group assessment and evaluation results and determining appropriate treatment.

  • Develop a plan of care including specific and measurable goals and group protocols to help individuals and groups address barriers to occupational participation via person and occupation-centered interventions.

  • Document goals applicable interventions and progress adhering to reimbursement and practice act standards.

  • Collaborate with treatment team for care coordination and incorporation of OT services in the individuals person-centered plan.

  • Promote occupational engagement and participation by selecting and utilizing appropriate screens assessments and interventions for individuals groups and populations based on the occupational profile and desired functional outcomes.

  • Address both mental and physical impairments impacting occupational performance as well as contextual barriers and recommend compensatory strategies to include but not limited to recommendations and trainings for assistive technology adaptive equipment and durable medical equipment.

  • Educate an individuals family and/or employer about how to accommodate and support occupational engagement.

  • Collaborate with treatment team for discharge planning to include recommendations for follow-up services and community supports.

  • Attend and actively participate in meetings and trainings as required.

  • Maintain certification/licensure in all agency state and federal training requirements.

  • Maintain all necessary documentation and records in a timely manner. This may include the use of an Electronic Health Record system (EHR).

  • Demonstrate knowledge of and comply with all agency policies and procedures as well and service definitions related to specific programs.

  • Complete all other relevant responsibilities as assigned by the supervisor.

  • Driving and travel may be required.

  • Additional Job Description Degree in Occupational Therapy required.

  • Completion of an accredited occupational therapy educational curriculum.
