Position Summary

The Occupational Therapist evaluates plans and implements individualized treatment programs that help patients improve functional independence and quality of life. This role collaborates closely with physicians nursing staff rehabilitation professionals patients and families to develop effective treatment plans and achieve optimal outcomes.

Occupational Therapist Responsibilities
  • Evaluate patients within required regulatory timeframes following physician referral
  • Develop individualized treatment plans and establish measurable goals
  • Provide occupational therapy services in accordance with physician orders and professional standards
  • Assess patients abilities to perform activities of daily living (ADLs)
  • Supervise Occupational Therapy Assistants (COTAs) aides and students as appropriate
  • Collaborate with physicians nursing staff and interdisciplinary team members regarding patient progress and care plans
  • Participate in care conferences rehab meetings and discharge planning
  • Educate patients families caregivers and staff on treatment plans and therapy techniques
  • Recommend adaptive equipment assistive devices and durable medical equipment as needed
  • Complete accurate and timely documentation in compliance with federal state and facility guidelines
  • Maintain therapy equipment and report maintenance or repair needs
  • Promote a positive compassionate and patient-focused rehabilitation environment
Qualifications
  • Current and active California Occupational Therapist License required
  • Bachelors degree in Occupational Therapy required
  • Masters or Doctorate in Occupational Therapy preferred
  • Skilled nursing rehabilitation post-acute or long-term care experience preferred
  • Strong clinical assessment and treatment planning skills
  • Excellent communication and interpersonal abilities
  • Experience with PointClickCare and Casamba preferred
  • Ability to work effectively in a fast-paced team-oriented environment
  • Commitment to delivering exceptional patient-centered care
