Occupational Therapist (OT) Home Health Buffalo NY PRN & Part-Time South Buffalo
Stern at Home Therapy is hiring Occupational Therapists (OTs) for one flexible opening serving patients throughout the South Buffalo NY area.
We currently have an opportunity covering the South Buffalo region allowing you to build a caseload nearwhere you live. Choose between part-time W-2 employment with benefits or a flexible PRN opportunity based on your availability.
Treat one patient at a time manage your own schedule and provide individualized care without the double-booking productivity quotas and clinic burnout common in traditional outpatient settings.
What Youll Do:
As an Occupational Therapist with Stern at Home Therapy you will evaluate and treat a primarily geriatric patient population in their homes throughout either the North or South Buffalo service area.
Responsibilities include:
- Perform occupational therapy evaluations and develop individualized plans of care.
- Provide one-on-one treatments based on each patients functional needs and goals.
- Manage individualized treatment plans with clinical autonomy.
- Help patients improve independence safety mobility and their ability to perform daily activities.
- Complete accurate and timely clinical documentation.
- Communicate with patients families physicians and members of the clinical team.
- Coordinate with our administrative team regarding scheduling patient care and logistics.
- Build a caseload based on your preferred geographic area and availability.
Compensation:
Part-Time W-2:
- $55 per follow-up visit.
- $65 per 4-unit evaluation.
PRN:
- $62 per 4-unit follow-up visit.
- $75 per evaluation.

Qualifications:
- An active Occupational Therapist (OT) license in New York is required.
- Reliable transportation required.
- Strong clinical communication and time-management skills.
- Ability to work independently in a home-based setting.
- Home health experience is preferred but not required.
