Responsible for emergency call rotation/on-call for one week every 6 SummaryThe function of the nurse is to promote optimum health and wellbeing of adults with intellectual and developmental disabilities in group home and apartment settings. The nurse works as a health advocate and supports the individual and interdisciplinary team. The nurse assists direct care staff in meeting regulations & licensing required for 245D state and federal compliance. All employees are expected to center model and champion Hammers core values: Person-Centered Relational Opportunistic and Duties and ResponsibilitiesNursing AssessmentsMonitor the health status of the individuals who have been assigned to your changing health conditions to appropriate team health related concerns for individuals in a timely appropriate medical and remedial services and incorporate recommendations into the a health care plan and incorporate it into the CSSP (Coordinated services & support plan) for ICF/DD Conditions and Wellness Education Teach and counsel the individuals we serve family members and staff in the areas of health and disease andpromoting self-responsibility and independence whenever promptly to calls on the nurse emergency cell phone on a rotating for an awareness of the needs and changing lifestyles on the developmentally disabled persons in the and Safety Standards ProtocolsAbide by HIPPA and PHI in the periodic review of the CSSP as a member of the interdisciplinary in instructing the staff in medical related orientation classes and annual continuing education in infection control discussions and implementation across the all health service policies and procedures in compliance with the state and/or with the development of individual/household health or medical related protocols and and participate in regular staff meetings/in services deemed effectively and work cooperatively with co-workers and administrative staff as backup nursing support when assigned nurse is unavailable to assist within their assigned paperwork promptly accurately and thoroughly with attention to Knowledge and QualificationsLicensed as a Registered Nurse in good standing with the State of Minnesota. Bachelor of Science in Nursing (BSN) preferred. Demonstrated proficiency in managing health services for people who have intellectual disabilities and other related conditions. Valid drivers license and driving record that meet the requirements of Hammer Residences insurance receive a MN DHS Background study clearance and show a negative Tuberculosis test written and verbal communication skills including fluency in English and proficiency with Microsoft Office (especially Outlook and Excel) and strong organizational skills; frequently organizes multiple projects at once good time management close attention to detail autonomy based on individuals health needs and proven ability to achieve assigned a strong commitment to confidentiality and customer ability and commitment to working with individuals and teams that are mixed across lines of difference such as race gender-identity sexual orientation religion ability age class and immigrant to a strong ethical code in compliance with organizational mission and demands include frequent use of a telephone frequent use of repetitive motion activities including typing on a computer keyboard andmonitor laptop or scanner andthe ability to work for several hours at a time at a desk and to bend and move to perform nursing head to toe assessment evaluations lifting and to be able to complete skilled nursing $54500.00/annuallyRN$63800.00/annuallyAnnual increase of $1000 for BSN completion
