Were seeking a Part-Time Family Nurse Practitioner (FNP) to provide comprehensive primary care focused on prevention early intervention and long-term wellness.

Youll care for patients ages 2 and older managing acute and chronic conditions developing individualized treatment plans and empowering patients through education and ongoing support.

Position Hours: 18 hours per week

Clinic Operating Hours: MondayThursday 7:00 AM5:00 PM

Work Schedule: To be determined within clinic operating hours.

• Provide comprehensive primary care through in-person and virtual visits as clinically appropriate
• Assess diagnose and manage acute and chronic health conditions for patients ages 2
• Develop individualized treatment plans and prescribe appropriate medications and therapies
• Promote preventive care wellness and chronic disease management through patient education
• Collaborate with physicians medical assistants and other clinicians to support coordinated patient-centered care
• Maintain accurate clinical documentation and adhere to applicable medical regulatory and HIPAA requirements
• Contribute to a positive collaborative clinic environment focused on quality care and patient outcomes

• Active unrestricted Nurse Practitioner license in the state of practice
• Current Family Nurse Practitioner (FNP) board certification
• Active DEA registration
• Experience providing primary care to patients ages 2 and older
• 2 years of primary care experience preferred
• Knowledge of HIPAA requirements and commitment to patient confidentiality
• Ability to successfully complete required credentialing
• Commitment to preventive care chronic disease management and patient education
