Description:
Impresiv Health is seeking a Nurse Practitioner to join an interdisciplinary care team providing comprehensive medical care and chronic disease management to a focused panel of patients living with chronic kidney disease (CKD) and other complex conditions.
This role focuses on preventive care chronic disease management patient education and care coordination through a value-based relationship-centered approach. The Nurse Practitioner will work closely with physicians Social Workers Care Navigators specialists and community resources to ensure care is coordinated connected and centered around the whole person.
Patients are seen in their homes virtually or by phone providing the opportunity to build meaningful relationships and gain a deeper understanding of each patients unique health needs.

Impresiv Health is seeking a Nurse Practitioner to join an interdisciplinary care team providing comprehensive medical care and chronic disease management to a focused panel of patients living with chronic kidney disease (CKD) and other complex conditions.
This role focuses on preventive care chronic disease management patient education and care coordination through a value-based relationship-centered approach. The Nurse Practitioner will work closely with physicians Social Workers Care Navigators specialists and community resources to ensure care is coordinated connected and centered around the whole person.
Patients are seen in their homes virtually or by phone providing the opportunity to build meaningful relationships and gain a deeper understanding of each patients unique health needs.
Schedule and Work Model:
- Monday through Friday 8:00 AM to 5:00 PM with flexibility based on patient needs
- No routine weekends or overnight shifts
- Limited after-hours calls
- Approximately 5 to 6 patient visits per day
- Combination of in-home visits telehealth and care coordination
- Daily collaboration with patients and interdisciplinary care team members to coordinate comprehensive high-quality care
- Manage a small panel of patients with CKD and other complex chronic conditions alongside an interdisciplinary care team
- Conduct comprehensive health assessments medical histories physical examinations and ongoing clinical evaluations
- Diagnose treat and manage acute and chronic conditions within scope of practice
- Order interpret and review laboratory and diagnostic testing
- Prescribe and manage medications in accordance with state regulations
- Develop individualized care plans addressing each patients medical behavioral and social needs
- Educate patients and caregivers on disease prevention medication adherence lifestyle changes and treatment plans
- Coordinate care across primary care providers specialists hospitals skilled nursing facilities home health hospice and community organizations
- Participate in regular interdisciplinary care team huddles
- Monitor patient progress and adjust care plans to improve outcomes and reduce avoidable hospitalizations
- Document clinical encounters accurately and on time
- Support quality improvement initiatives and evidence-based practice
- Maintain patient confidentiality and comply with company policies and regulatory requirements
- Provide after-hours telephonic support as needed
- You are comfortable working independently while collaborating closely within a team
- You are comfortable delivering care both in person and via telehealth
- You have a genuine passion for relationship-centered value-based care for patients with chronic illness
- Active Nurse Practitioner or Physician Assistant license in Arizona or Florida
- Board certification
- Ability to meet clinician credentialing requirements including a background check
- Valid drivers license reliable transportation and the ability to travel to patient homes
- Strong clinical assessment communication and critical-thinking skills
- Comfort working independently while collaborating closely within a team
- Comfort delivering care both in person and via telehealth
- A genuine passion for relationship-centered value-based care for patients with chronic illness
- Experience in primary care internal medicine geriatrics home-based care urgent care or value-based care
- Experience caring for patients with multiple chronic conditions
