- $125000 to $170000 Annual Income
- $105000 to $125000 Base Salary
- $20000 to $45000 Annual Bonus Income paid quarterly

Their services include among others primary care psychiatric care post-acute care and transitional care.
POSITION DESCRIPTION:
The nurse practitioner will serve as the primary care provider for geriatric patients located within a single skilled nursing facility (SNF) in Mentor.
Providers see a variable number of patients per day dependent upon patient needs.
Specific duties of the role include but are not limited to:
- conduct health assessments to identify patient needs
- develop personalized care plans
- regularly evaluate patient conditions and responses to treatment
- adjust treatment plans as necessary
- perform minor procedures and wound care as needed
- educate patients and families on health management disease prevention etc.
- provide guidance on lifestyle modifications and self-care practices
- work with physicians nurses and other professionals to coordinate care
The practice uses a mobile-based EHR with AI functionality to reduce administrative time.
On-call is phone triage only and providers rotate the responsibility evenly with some flexibility in scheduling available.
REQUIREMENTS:
- Ohio NP license
- DEA
- new grads with backgrounds in home health LTC etc. are encouraged to apply
- no experience required
- full training provided
