About the Role:
As a Nurse Practitioner in post-acute care you will provide in-person care to patients in skilled nursing and rehabilitation this role you will partner with facility teams and Trumen-affiliated physicians to manage chronic and acute conditions support transitional care planning and contribute to quality-improvement initiatives within your designed facilities.
Core Responsibilities:
- Collaborate with Trumen physicians and facility medical directors to coordinate patient care
- Conduct comprehensive clinical assessments and manage treatment plans
- Support transitional care planning to help prevent avoidable hospital readmissions
- Reconcile medications and provide patient education consistent with scope of practice
- Foster positive working relationships with facility leadership clinical staff and care coordinators
- Collaborate as applicable Trumen-affiliated value-based and quality improvement initiatives
- Serve as a liaison between Trumen and designated skilled nursing facilities (SNFs)
- Maintain accurate and timely documentation within the EMR
- Maintain an on-site presence consistent with your agreed schedule and facility needs
Qualifications:
- Active Nurse Practitioner license in the state of Texas
- Minimum of 1 year experience as a Nurse Practitioner preferably in a post-acute SNF or geriatric care setting
- Ability to work independently while effectively collaborating with the care team
- Strong interpersonal skills and a passion for caring for complex patients
- Reliable transportation to assigned facilities
