Minimum qualifications:

  • Bachelorsdegree in Nursing

  • Three years of experience in an acute care setting. One year of experience in medical surgical or radiationoncology(foroncologyspecialty).

What the Nurse Navigator will do:

  • Completes a comprehensive health assessment of the patients physical psychological social .

  • Assistsinorganizingandcoordinatingtimelypatientappointments.

  • Understands and develops goals treatmentplanand provider options in concordance withevidence basedstandards and clinicaltrials.

  • Worksdirectlywithphysicianleadtoreviewdiagnosticstudiesclinicalhistoryandcourseoftreatmenttoidentifymostappropriatenextcourse ofcare.

  • Collaborateswithmultidisciplinaryteammembersinperformanceimprovementactivitiesincludingassessmentplanningimplementationandevaluationofclinicaloutcomesand processesfor patients.

  • Usescritical thinking andproblem solvingskills to analyze and synthesize data and formulatean appropriate casemanagementplantoaddressthe patientsindividual andfamilyneeds.
