The Cape May Court House Orthopaedic Nurse Navigator serves as a key clinical resource and patient advocate guiding patients throughout their orthopaedic surgical journey. This role coordinates pre-operative and post-operative care facilitates communication between patients and the care team provides education and support assists with discharge planning and ensures timely follow-up to promote optimal outcomes. The Nurse Navigator works collaboratively with surgeons advanced practice providers rehabilitation services and other healthcare professionals to deliver seamless patient-centered care and enhance the overall patient experience.
Minimum 3-5 years of nursing experience required; orthopedic musculoskeletal surgical or specialty practice experience preferred. Demonstrated expertise in patient education care coordination navigation multidisciplinary collaboration and clinical triage. Proficiency with EHR documentation.
BSN required
Current NJ-RN License or if working in an office in another state that applicable state license.
