Salary: $95000 - $113000 annually depending on experience
Help guide residents through successful rehabilitation and transitions of care in a leadership role that combines clinical expertise collaboration and resident advocacy.
Lutheran Home is seeking an experienced Registered Nurse to oversee care coordination within our skilled nursing and rehabilitation this role you will partner with interdisciplinary teams to support resident recovery discharge planning and quality outcomes while ensuring exceptional clinical care.
- Coordinate care for residents receiving short-term rehabilitation and skilled nursing services
- Assist with admission assessments care planning and discharge preparation
- Collaborate with physicians therapists nursing staff social services and families to support individualized care
- Identify and address changes in resident condition to promote positive outcomes and reduce avoidable hospital transfers
- Provide guidance and education to residents families and team members regarding treatment plans and recovery goals
- Monitor documentation care delivery and regulatory compliance standards
- Support operational and clinical initiatives within the rehabilitation program
- Participate in an on-call rotation to assist with resident care needs
- Current Illinois Registered Nurse (RN) license
- Minimum of two years of nursing experience in skilled nursing rehabilitation post-acute care or long-term care
- Experience coordinating resident care and collaborating across multiple disciplines
- Strong communication and organizational skills
Preferred
- Nurse leadership supervisory or care management experience
- Knowledge of post-acute rehabilitation and transitional care practices
- Ability to effectively prioritize and manage competing responsibilities
