Jobposition description:

This positionensures the highest degree of quality care by planning organizing and leadingthe overall operation of our care services in accordance with resident needsgovernment regulations and our internal policies and procedures.

EssentialResponsibilities:

• Oversee care coordination for allresidents including but not limited to; coordinates service plan activatesappropriate resources and communicates with the physician or pharmacy regardingmedication changes/issues follows up on residents admitted to the hospital andcompletes necessary paperwork.
• Completes medical assessments of eachnew admission and ongoing changes in condition. Provides recommendations toExecutive Director on new admissions.
• Manages medication administrationmedication records and medication documentation in the MARs. Maintains residentdocumentation medical management documentation and care plan documentation.
• Leads and delegates nursing tasks todirect care staff in compliance with the Nurse Practice Act and regulatoryrequirements.
• Continually seeks out and directquality improvement activities that focus on wellness clinical managementmedication management risk management staff training and safety/surveypreparedness.
