Job Summary
These RN Care Manager positions are located in the Primary Care Service Line.
This nurse provides gender-specific direct Veteran care and care coordination as it pertains to Veterans and their care within the Primary Care Department.
Duties include but are not limited to:
1. Leads and organizes delivery of care to ensure continuity of care and peer accountability for practice in alignment with the ANA Standards of Nursing Practice.
2. Applies advanced clinical knowledge/judgment to lead staff involvement in planning decision-making and evaluating outcomes beyond the immediate practice setting.
3. Applies nursing expertise to lead improvement with demonstrated outcomes beyond the immediate practice setting.
4. Demonstrates ability to be self-directed in using the nursing process to improve the outcomes of care at the program/service level.
5. Applies collaborative team approach in identifying analyzing resolving problems and sustaining improvements.
6. Applies problem solving skills beyond the unit level leading to improved care/outcomes.
7. Leads and directs others in organizing the delivery of care to assure continuity of care and peer accountability for practice.
8. Evaluates program/process or medical center policies/procedures and reports effectiveness or provides leadership analyzing and addressing ethical legal issues that impact patients and staff and the medical center.
9. Demonstrates knowledge and compliance of Ethical Resolution Process.
10. Assumes responsibility and demonstrates accountability for all professional nursing actions.
11. Provides considerate care respecting patients/staffs personal values and belief system in provision of care.
12. Supports and enhances patient self-determination.
13. Safeguard privacy and maintains confidentiality of all patient information including electronic and print.
14. Serves as a resource and advocate for the protection of patient/family rights.
15. Initiates and participates in consultations by analyzing ethical issues appropriate theoretical principles and provides guidance as appropriate.
16. Models ethical conduct with other staff.
17. Analyzes available resources; identifies risk areas which include budgetary staffing supplies and equipment or availability of data/information
18. Evaluates options available and directs a course of action which will result in the most favorable patient program or organizational outcomes
19. Establishes patient care priorities that ensure safe efficient and effective patient care.
20. Provides care based on patient needs that can be delivered in a safe efficient and cost-effective manner.
21. Evaluates and assigns care based on staff competencies and priorities ensuring effective patient care with optimum use of all assigned resources.
22. Identifies all aspects of patient care delivery and utilizes appropriate resources to enhance patient outcomes.

- Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).

Basic Requirements:
- English Language Proficiency. In accordance with 38 U.S.C. 7403(f) no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
- Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE); OR Individuals attending a masters level bridge program in nursing who have completed coursework equivalent to a bachelors level degree in Nursing may have opportunity to become registered as a nurse with a state licensing board prior to completion of the bridge program. Upon achievement of a State license the individual may be appointed on temporary basis and later converted to a permanent appointment upon successful completion and graduation from the bridge program. (Reference VA Handbook 5005 Appendix G6); OR In cases of graduates of foreign schools of professional nursing possession of a current full active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing to warrant an appointment as a Nurse who has completed an associated degree/entry level Nursing education program. Credit for foreign nursing education higher that associate degree/entry level requires a formal degree equivalency validation from a recognized equivalency evaluation accepted by VA such as International Consultants of Delaware (ICD).
- Current full active and unrestricted registration as a graduate professional nurse in a State Territory or Commonwealth (i.e. Puerto Rico) of the United States or the District of Columbia. Graduate Nurse Technician (GNT) Exception: Candidates who otherwise meet the basic education requirements but do not possess the required licensure may be appointed at the entry step of the grade and level applicable to the completed nursing education as a GNT on a 120-day temporary appointment while actively pursuing licensure (may be extended up to two years on a case-by-case-basis.)
