CBI provides residential outpatient inpatient patient-centered medical homes medication-assisted treatment and crisis services to individuals experiencing crisis opioid use disorder homelessness and mental is the premiere non-profit fully integrated healthcare provider of substance use and behavioral health programs in Arizona including prevention education and treatment using cutting-edge nationally recognized evidence-based SummaryThe Navigator IIs primary role is to enhance the likelihood of success identify and reduce barriers for clients and connect them with needed services and resources. The Navigator II will work with individuals who have been determined as in need of additional support and engagement and will be responsible for conducting outreach engagement case management and ongoing wrap around supportive services. The Navigator II will also map out the most appropriate next steps for the individual who may need assistance in accessing various services to achieve ongoing success. The Navigator II is part of a multi-disciplinary team made up of internal and external stakeholders. The Navigator II will document and track required patient interactions according to the Arizona Administrative Code (AAC) Arizona Department of Health Services/Department of Behavioral Health Services (ADHS/DBHS) Provider Manual Commission on Accreditation of Rehabilitation Facilities (CARF) Community Bridges Policies and Procedures and specific for the this role you will be on the front lines of delivering lifesaving support to vulnerable populations affected by extreme heat. You will collaborate with community members local service providers and internal teams to distribute resources share information and connect people in need with the appropriate services. This position requires a strong commitment to community engagement empathy and responsiveness to the urgent needs of individuals experiencing or at risk of heat-related health diploma or GED is required.
