Navigator lThis is a seasonal role active during peak heat periods from May to October. However there may bepotential for continued employment if suitable roles become available within the agency. (CB I) is an integrated behavioral healthcare agency offering a variety of different programs throughout Arizona. CBI provides residential outpatient inpatient patient-centered medical homes medication-assisted treatment and crisis services to individuals experiencing crisis opioid use disorder homelessness and mental is the premiere non-profit fully integrated healthcare provider of substance use and behavioral health programs in Arizona including prevention education and treatment using cutting-edge nationally recognized evidence-based SummaryThe primary role of the Navigator I is to enhance the likelihood of success identify and reduce barriers for clients and connect them with needed services and resources. The Navigator will work with individuals who have been determined as in need of additional support and engagement and will be responsible for conducting outreach engagement case management and ongoing wrap around supportive services. The Navigator I will also map out the most appropriate next steps for the individual who may need assistance in accessing various services to achieve ongoing success. The Navigator I is part of a multi-disciplinary team made up of internal and external stakeholders. The Navigator I will document and track required patient interactions according to the Arizona Administrative Code (AAC) Arizona Department of Health Services/Department of Behavioral Health Services (ADHS/DBHS) Provider Manual Commission on Accreditation of Rehabilitation Facilities (CARF) Community Bridges Policies and Procedures and specific for the this role you will be on the front lines of delivering lifesaving support to vulnerable populations affected by extreme heat. You will collaborate with community members local service providers and internal teams to distribute resources share information and connect people in need with the appropriate services. This position requires a strong commitment to community engagement empathy and responsiveness to the urgent needs of individuals experiencing or at risk of heat-related health diploma or GED is required. Associates Degree (or higher) in a field related to Behavioral Health is preferred but not required of this be in recovery from Alcohol or Drugs and/ormental health for equal to or greater than 1 consecutive year.1-3 years of full-time Health Care related work experience where his or her prior experience would provide adequate exposure to both behavioral and medical crisis situations is preferred but not required.1-3 years of substance abuse/mental health recovery-oriented service is preferred but not AZ Drivers License valid and good 39-month Motor Vehicle Fingerprint Clearance card or good cause exemption Offers an excellent benefits package!Generous PTO accrual (5 weeks!)Medical Dental Vision Disability Life Supplemental plansHospital indemnity/ Critical IllnessPet InsuranceDependent Care Savings Health Care Savings401K with employer match - 100% vested upon enrollmentWellness programsTuition Reimbursement and Scholarship Programs incentives and more!Very Completive pay ratesCBI is growing an expanding our services!We are experience tremendous growth in this time.
