Summary
Provides case management services under the Medicaid Alternative Care (MAC) Tailored Supports for Older Adults (TSOA) Family Caregiver Support Program (FCSP) and other Senior and Independent Living Services programs in accordance with Washington Administrative Code (WAC) state policy and program-specific requirements.

General Responsibilities (essential functions):
- Complete the initial interviews with clients and their families to assess the customers eligibility for services.
- Assemble and guide group discussions and decision-making sessions among relevant professionals and program representatives clients and their families and significant others to formulate goals and design an integrated intervention plan.
- Monitor adherence to the plan and manage the flow of accurate information within the action system to maintain a goal orientation and coordination momentum.
- Provide follow-along to the client and his or her family to speed up identification of unexpected problems in service delivery
- Perform other related duties as assigned.
Required Education or Experience:
Per Washington State Aging & Long-Term Support Administration Policy Case Managers will meet at least the following minimum education and experience requirements:
- Four years of paid full time on-the-job social service experience in providing care management and long-term support and services to vulnerable adults such as providing one-on-one in-person customer service developing care plans for aged blind or disabled individuals and caseloads and a high school diploma or its equivalent; or
- Three years of paid full time on-the-job social service experience providing one-on-one in-person customer service developing care plans for aged blind or disabled individuals and caseloads a high school diploma or its equivalent and are bilingual or bicultural with such experience; or
- Two years of experience with other DSHS information systems used by Case Management staff such as Comprehensive Assessment and Reporting Evaluation (CARE) tool Barcode ProviderOne QA Monitor or ALTSA reporting and college courses an Associates degree or its equivalent; or
- Two years of paid full time on-the-job social service experience and Bachelors degree; or
- One year of paid full-time on-the-job social service experience and a Bachelors degree in social services behavioral or health sciences; or
- Social service experience and a Masters degree.
One year full-time equivalent verifiable hours through an internship or volunteer work in social services may be substituted for one year of paid social service experience in providing care management and long-term support and services to vulnerable populations. Part-time hours may be considered if the hours total one year full-time equivalent.
Individuals functioning as Case Managers prior to August 281991 who have performed competently as determined by their performance evaluations may be deemed Case Managers from a Washington State Area Agency on Aging when they meet the following educational requirements:
- High School diploma or its equivalent and four years of paid on-the-job social service experience prior to August 28 1991; and
- Completion of state-sponsored CORE training.
Required Certification and Licenses:
- Valid drivers license in state of residence.
- Auto insurance in the amount required by the State of Washington.
- Acceptable completion of a criminal history background check.

Full Wage Range: Additional wage progression may be available over time under the organizations established compensation structure. The full established pay scale for this position is $21.65 $57.73 per hour; however placement above the starting wage range is not available at hire.
