Memory Health Program Coordinator
Schedule: Part-Time 16 Hours Tuesday & Thursday
Position Summary
The Memory Health Coordinator supports and coordinates memory care services for Residentswith cognitive changes promoting their physical social and emotional well-being. This roleassists in delivering memory-related programs provides guidance to team members andcollaborates with the Manager of Memory Health Services to ensure best practices in dementiacare across the continuum.

  • Delivers person-centered evidence-based programming that supports cognitive andemotional well-being
  • Provides and receives constructive feedback to continuously improve services
  • Advocates for Residents and communicates effectively with families Team Membersand other departments
  • Adapts programming to meet Resident needs and provides Team Members with toolsand strategies for effective memory care
  • Monitors and reports on changes in Resident behavior or condition to appropriate TeamMembers
  • Guides Team Members in promoting positive outcomes for Residents
  • Collaborates with the Memory Health Manager and other departments to ensurecoordinated care
  • Builds trusting relationships with Residents and families to support quality of life
  • Assists in implementing maintaining and evaluating memory care programs
  • Prepares reports and documentation to inform decision-making and support teamcommunication
  • Identifies program needs and contributes to ongoing improvements
  • Creates a welcoming personalized experience for each Resident
  • Promotes engagement by tailoring activities and interactions with individual interestsand abilities
  • Meets department uniform appearance and grooming requirements
  • Complies with all OSHA safety regulations applicable local laws regarding health safetyor environment
Required Skills and Abilities
  • Effective communicator in one-on-one and small group settings
  • Strong organizational time management and record-keeping skills
  • Demonstrates sound judgment and decision-making in routine situations
  • Work professionally with people from a wide variety of cultural and socio-economicbackgrounds
  • Must possess the ability to deal tactfully with Team Members Residents familymembers visitors and the general public
  • When working in Resident areas must be able to communicate effectively in English
  • Complies with protection of health information (HIPAA) safeguards
  • Attends and participates in in-service educational classes Healthcare Academy and staffmeetings as directed
Education and Experience
  • High school diploma or equivalent required
  • Experience in senior living and/or dementia care preferred
  • Willingness to complete dementia-related training
  • Uses basic Microsoft programs (Word Excel) email and office equipment
  • Basic knowledge of dementia and cognitive decline
Satisfactory completion of a background investigation is required for this position.

Pay Range: $25/hour
Individual compensation within this range is based on a candidates specific qualifications including education overall experience relevant experience and specific skills. We strive to ensure that compensation is competitive fair and reflects the value each candidate brings to the role.
