Title: Medical Social Worker (MSW)

Schedule: MondayFriday 8am - 4pm

Location: Home Care - Lakes Region (Wolfeboro Meredith Laconia areas)

Pay: $35.00 - $44.00/hour Mileage Reimbursement at the Federal Rate

Reporting to the Director the MSW provides age-appropriate counseling education and support to patients and families in the home setting using standard social work practices. This role partners closely with patients caregivers physicians and the interdisciplinary care team to develop and coordinate individualized plans of care.

• Assess patient and family needs and develop individualized plans of care
• Provide counseling behavioral health interventions education and community resource support
• Coordinate care with physicians and interdisciplinary team members
• Document patient progress interventions and outcomes in accordance with agency standards
• Conduct hospice information visits for home care patients when requested
• Support patient and caregiver education and discharge planning
• Mentor and support new MSW staff and students

1. Masters degree in social work from an accredited program (Council on Social Work Education) OR bachelors degree in social work with at least one year in healthcare care setting.
