As our CC Master Social Worker you will plan implement and organize social work resources and evaluate and provide treatment regarding mental health issues for patients facilitating their linkage to community resources.

Every day you will assess patient needs deliver mental health treatment as appropriate develop comprehensive care plans and ensure seamless connections to relevant community support systems all in coordination with continuing care.

To be successful in this role you will possess an MSW clinical expertise in mental health treatment strong resource navigation skills and the ability to effectively plan implement and evaluate social work interventions to support continuing patient care and community integration.

• Develop and implement patients plan of care and observe outcomes in accordance with social work standards and in collaboration with the interdisciplinary team.
• Assess patients physical psychosocial cultural and spiritual needs through observation interview review of medical records and clinical data.
• Evaluate patients response and intervene to ensure optimal patient outcome.
• Promote advocate and collaborate to protect the health safety and rights of each patient.
• Demonstrate competency in a variety of therapeutic/diagnostic interventions.
• Use quality measures to improve performance and accountability for patient outcomes patient experiences and safe delivery of care.

Required

• Masters Degree in Social Work
  
• LSW required
• Basic Life Support (BLS) from American Heart Association for facilities with an Automated External Defibrillator (AED) preferred for facilities without an AED
  
• Valid Drivers License
