Job Summary and Responsibilities

As a Manager Care Coordination you will oversee and lead the daily operations of the Care Coordination department ensuring high-quality patient-centered care transitions.

Every day you will manage and mentor Care Coordination RNs monitor performance ensure regulatory compliance and collaborate with interdisciplinary teams to optimize patient flow and effectiveness.

To be successful you will demonstrate strong leadership strategic planning and clinical expertise driving continuous improvement and optimal patient care coordination for organizational goals.

Job Requirements

Bachelors of Nursing upon hire or 1-3 years experience equivalent required and

Minimum of three (3) years experience in Care Coordination or Case Management.

Registered Nurse: CA (RN:CA) required

Certified Case Manager (CCM) preferred
