Location:Bloomfield New Mexico
Schedule: Day Shift 36 Hours Guaranteed 13 Weeks
Shift Timing: 5:00 AM 5:30 PM (30-minute meal break)
Working Days: Monday Tuesday Wednesday

Job Summary
A skilled nursing and rehabilitation facility in Bloomfield New Mexico is seeking an experienced LPN/RN for a 13-week local contract assignment. The role involves providing direct patient care in a long-term care and rehab setting ensuring safe high-quality and patient-centered nursing care.
Key Responsibilities
- Provide direct nursing care to residents under clinical supervision
- Administer medications and treatments as prescribed
- Monitor assess and document patient conditions and vital signs
- Support care plans and rehabilitation goals
- Assist with activities of daily living (ADLs) as needed
- Communicate patient condition changes to healthcare team
- Ensure patient safety comfort and dignity at all times
- Maintain accurate clinical documentation
- Follow infection control and facility safety protocols
- Collaborate with interdisciplinary care teams
Required Qualifications
- Active LPN or RN license required
- Prior experience in long-term care or rehabilitation preferred
- Strong clinical assessment and medication administration skills required
- Ability to monitor and document patient conditions accurately required
- Knowledge of infection control and safety procedures required
- Strong communication and documentation skills required
- Ability to work in a structured healthcare environment required
