The Memory Care Center at Well-Spring Solutions is seeking a compassionate Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our team. This is a unique opportunity to provide nursing care and support to older adults living with Alzheimers disease dementia and other memory-related conditions in a warm engaging adult day setting.

Unlike many nursing positions this role offers a predictable schedule with no nights weekends on-call responsibilities or holidays.

Position Summary

As the Health Coordinator you will serve as the clinical leader within the program and help ensure participants receive high-quality person-centered care.

Essential Responsibilities

• Complete pre-admission health assessments
• Develop implement and update individualized care plans
• Monitor participant health status and respond to changes in condition
• Communicate with physicians specialists caregivers and family members
• Supervise medication administration first aid and emergency care
• Coordinate referrals and community healthcare services
• Maintain accurate nursing documentation and health records
• Provide health education to participants caregivers and staff
• Support state certification and regulatory requirements
• Assist with staff training emergency preparedness and program operations

Qualifications

• Current North Carolina RN or LPN license
• Current CPR and First Aid certification
• Minimum of two years of nursing experience preferred
• Experience in memory care dementia care geriatrics adult day services home care assisted living long-term care or community health preferred
• Strong assessment communication and organizational skills
• Passion for serving older adults and supporting family caregivers
