POV: Youre not sitting behind a deskyoure out in the community in someones home and right alongside them as they work toward their goals. You might help someone plan their day build relationships with family and friends discover a new community activity connect with important services or create tools that make everyday life a little easier. When needed youll also provide hands-on personal support with things like bathing toileting or feedingwith dignity patience and respect.
Some days might mean helping someone reach a personal goal. Other days might mean building confidence strengthening connections or simply trying something new. Youre not there to do life for someoneyoure there to support them in creating the life they choose.
And yes this position includes overnight support giving you the opportunity to make an impact around the clock.

- Provide support to clients in their home and community including overnight assistance.
- Assist with development and implementation of annual Supported Living Plan (SLP) goals and objectives.
- Participate in the development of a circle of support both paid and unpaid support to build relationships with family friends neighbors and other community members.
- Develop individualized adaptive support materials as needed.
- Coordinate and monitor other services (such as MTS Medi-Cal Social Security etc.).
- Provide assistance with personal support to include toileting bathing feeding etc. **if necessary
- Develop and expand clients participation in integrated community activities.
- Support & Communicate: Work closely with individuals supervisors and planning teams to support goals and keep everyone informed.
- Document & Stay Organized: Complete required documentation progress notes timesheets mileage and other records accurately and on time.
- Get Out & Make an Impact: Drive or use community transportation to help individuals pursue their goals build independence and participate in their community.
Other Requirements: Must also be hired on by IHSS as a part of clients support; personal care is required
