Rehab Without Walls is seeking an experienced Licensed Clinical Social Worker (LCSW) to join our Home & Community Program in Seattle. This is a W2 PRN position (not 1099) offering flexibility with the backing and support of an established interdisciplinary care team.

Our innovative model brings outpatientlevel services directly to clients supporting recovery and reintegration in the environments where life actually happens.

What Youll Do:

• Provide counseling and psychosocial support in home and community settings
• Help clients navigate reallife transitions such as returning to work returning to driving and increased community participation
• Support emotional cognitive and behavioral adjustment following injury or illness
• Collaborate with an interdisciplinary team including OTs PTs and SLPs

What you will do: (responsibilities listed are included but not limited to)

• Performs comprehensive evaluations assessing the social emotional and support needs of the patients and their families
• Sets measurable objectives that are formulated in conjunction with the rehab team
• Assists the physician and other health team members in understanding the significant social and emotional factors related to the patients health problems
• Instructs/counsels patients and families in treating and coping with social and emotional response connected with illnesses
• Participates in care coordination activities and acts as a resource to other health team members in the identification and resolution of patient needs
• Identifies and assists the patient/ and or family to utilize appropriate community resources to achieve measurable objectives

What youll need:

• Masters Degree in Clinical Social Work (LCSW)
• Current Licensed Clinical Social Worker Licensure in the state of practice
• Minimum of one year experience in a rehabilitation setting serving individuals who have experienced a traumatic brain injury spinal cord injury or acute diagnosis
• CPR certification required
• Ability to lift 50 pounds
• Movingliftingor transferring of patients which may involve lifting of up to 100 pounds following safety procedures
• Duties require fine motor skillsvisual acuityand walking/standing for extended periods
• Additional physical requirements include:pushing/pulling bending/stooping reaching kneeling and positioning frequently at times
