As aLicensed Vocational / Practical Nurseyoullcontribute to our success in the following ways:

• Follow each patients individualized care plan and physician orders.
• Monitor patient progress and report changes or safety concerns to the care team.
• Help patients and caregivers learn self-care techniques to support their health.
• Work with the RN Case Manager to update care plans as patient needs change.
• Administer and document medications according to agency guidelines.
• Communicate with physicians patients families and the care team to ensure coordinated care.
• Help prevent avoidable hospitalizations and emergency room visits through proactive patient care.
• Deliver compassionate high-quality care that supports positive patient outcomes.

What is Required

• Current State License as a Licensed Vocational / Practical Nurse
• Graduate of an accredited Certificate Diploma or Associate School of Nursing
• 1 year of experience in a clinical care setting
• Ability to frequently lift push pull and support up to 50 pounds including positioning or transferring patients and moving equipment
• Experience in a hospice or home health environment is preferred
