Job Summary
The Licensed Social Worker functions as part of an interdisciplinary team and is responsible for recognizing managing and coordinating all aspects of medical social work across the continuum of inpatient outpatient emergency and discharge. This position may be assigned to any inpatient or outpatient department.
Job Responsibilities
- Serve as a contact and informational resource for designated patients. Act as a patient and or family advocate by assisting the patient in achieving autonomy and self determination to reach their goals.
- Help patients express their views and choices by eliciting preferences and priorities from the patient and family regarding treatment plan.
- Provide patients with self-management skills and educational information to be participants in their own treatment and recovery making healthier lifestyle choices to improve or stabilize their health.
- Conduct continual reassessment and reevaluation of the planning process assessing the appropriateness for the plan of care evaluate progress toward goals and ensure that the plan is implemented in a timely fashion. Collaborate with the physician and interdisciplinary healthcare network throughout the continuum of care.
- Review plan of care and expectations with the patient family physician and healthcare team members. Assist patients in problem solving potential issues related to the health care system financial or social barriers.
- Attend multidisciplinary care rounds to assist in both the coordination of patient care and discharge planning process. Informs team members of current patient status and post discharge care planning process.
- Help patients and their family/caregivers cope with the impact of disease chronic conditions and/or terminal illness. This may include conversations regarding palliative care hospice and grief/loss.
- Make reports to Department of Child Services and/or Adult Protective Services according to the policies and procedures of Riverview Health and the state of Indiana.
- Cross train with other multidisciplinary areas and social service providers within the hospital based on need.
- Other duties as assigned.
Education Requirements
- Minimum: Bachelors degree in Social Work (BSW); degrees in Counseling Psychology Marriage & Family Therapy or Human Services may be considered when paired with equivalent licensure.
Experience Requirements
- Minimum: One (1) year clinical experience in a healthcare or behavioral health setting
- Preferred: Three (3) years clinical experience in a healthcare or behavioral health setting
License/Certification Requirements
- Preferred:current licensure or license eligible in the State of Indiana as a Licensed Social Worker (LSW); Other state-recognized behavioral health licensure in counseling social services or human services will be considered.
