Job Summary

Title:Licensed Practical NurseProgram: Win West Womens ShelterSalary: $37.30 per hour ($48500)Schedule: Part-time Monday - Friday 9am - 2pmPosition Overview:Reporting to the Assistant Director of Nursing Embedded Services and Nurse Manager for Embedded Nursing for nursing related supervisions. Indirectly reporting to the Director of Nursing Operations (DNO). Reports to the Program Director of designated Shelter for client assignments daily tasks and workshop scheduling. The LPN will provide educational resources and workshops for chronic health conditions and serve as a health promotion agent to improve health outcomes within transitional and permanent housing Duties & Responsibilities:Management of medication system for all clients at shelter including-Receiving and reviewing medications-Assists with calling for refills -Assuring safe storage of medications-Ensuring that clients have appropriate supplies and access to medications-Monitoring client reactions for side effects and reporting to prescribing provider-Keeping Medication Self Administration logs destruction log controlled substance logProvides nursing screening of all new clientsAssists in connecting psychiatric medical and substance abuse problems to appropriate providersParticipates in weekly clinical meetings with medical staff shelter case management & administrative staffCommunicates regularly with on-site psychiatric and medical providers case managers and on-site operations & administrative staffCommunicates with off-site psychiatric providers as necessary and appropriateAdministers medications ordered by the providersAdministers injections ordered by providersProvides medication teaching as neededAssists clients with self-administer medicationsProvides ongoing health education and support to clients Maintains documentation in compliance with agency policies and proceduresAssist with tele-psychiatry operations as scheduled with tele-psychiatry provider at Project RenewalAssist with Primary Care Clinic Residential Substance Use Program coverage as neededQualificationsNew York State licensed LPNExperience working with homeless individuals preferredMust have excellent oral and listening skillsMust have the ability to interface with clients as well as all levels of staffMust have knowledge Microsoft Office especially Word and Excel
