Location: Philadelphia PA
Starting Pay Range:$32.00 per hour
Position Type: Full-Time (37.5 hours/week) Monday Friday Schedule / Rotating On-Call Coverage

We are seeking a compassionate and organized Licensed Practical Nurse (LPN) to support adults with intellecetual and developmental disabilities living in community homes throughout Philadelphia.
This is an excellent opportunity for nurses who enjoy care coordination helath education advocacy and building long-term relationships with the people they serve.
What Youll Do:
- Visit residents in community homes and monitor their health needs
- Review medical appointments physician orders and treatment plans
- Coordinate care with pysicians specialist pharmacies and support teams
- Train staff on health-related supports and medical changes
- Participate in interdisciplinary meetings and care planning
- Support hospital admissions discharges and follow-up care
- Complete nursing documentation and required assessments
- Participate in a rotating on-call schedule
Qualifications:
- Current Pennsylvania LPN License.
- Two (2) years of nursing experience preferred.
- Valid drivers license and reliable transportation
- Ability to obtain required background clearances
